Overview

Gentamicin + piperacillin-tazobactam is a combination of two antibiotics frequently used together for empirical therapy in severe infections, particularly in immunocompromised or febrile neutropenic patients. Gentamicin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, leading to cell death. Piperacillin is a ureidopenicillin (a type of beta-lactam antibiotic) that irreversibly binds penicillin-binding proteins and inhibits bacterial cell wall synthesis. Tazobactam is a beta-lactamase inhibitor that protects piperacillin from enzymatic degradation by beta-lactamases produced by some bacteria, thereby broadening its spectrum of activity. The combination provides synergistic coverage against a wide range of Gram-positive and Gram-negative bacteria, including Pseudomonas aeruginosa and many resistant organisms[4][5][6]. This regimen is commonly indicated for serious hospital-acquired infections such as pneumonia, intra-abdominal infections, sepsis, cellulitis, pelvic inflammatory disease, and as empirical therapy in neutropenic fever[3][4][5].
