Overview

GFB-024 is a peripherally restricted recombinant humanized monoclonal antibody that acts as an inverse agonist of the cannabinoid receptor 1 (CB1). It is being developed as a precision medicine for patients with severe insulin resistant diabetic nephropathy (SIRDN) and CB1 pathway overactivation. The drug aims to inhibit CB1 signaling in peripheral tissues, thereby protecting against podocyte and tubular cell injury in the kidney without central nervous system side effects. Preclinical studies have shown that elevated endocannabinoids activate CB1 in podocytes and metabolic organs, leading to mitochondrial dysfunction, proteinuria, and kidney damage; GFB-024 blocks this pathway. The drug has completed Phase 1 clinical trials evaluating safety, tolerability, pharmacokinetics, immunogenicity, and biomarker response in overweight/obese healthy volunteers and subjects with type 2 diabetes mellitus[4][8][5].
