Drug intelligence / Profile preview
GFH375 + gemcitabine + nab-paclitaxel is an investigational combination regimen being evaluated for the treatment of KRAS G12D-mutant solid tumors, particularly first-line metastatic pancreatic ductal adenocarcinoma (mPDAC). The regimen consists of GFH375 (also known as VS-7375), an orally active, potent, and highly selective small-molecule KRAS G12D (ON/OFF) inhibitor developed by GenFleet Therapeutics and Verastem Oncology, combined with the standard-of-care chemotherapy doublet gemcitabine and nanoparticle albumin-bound paclitaxel (nab-paclitaxel). GFH375 non-covalently binds to both the active (GTP-bound) and inactive (GDP-bound) states of the KRAS G12D mutant protein, blocking downstream MAPK signaling and tumor cell proliferation. Gemcitabine is a nucleoside analog that inhibits DNA synthesis, while nab-paclitaxel is a microtubule inhibitor. This combination is being evaluated in Phase Ib/II and Phase III clinical trials (such as TARGET-D 301) to improve outcomes and overcome resistance in patients with KRAS G12D-mutated pancreatic cancer.
