Ginkgo diterpene lactone meglumine

Ginkgo diterpene lactone meglumine is an injectable pharmaceutical preparation derived from the leaf extract of Ginkgo biloba. Its primary active ingredients are the terpene lactones ginkgolide A, B, and K in a typical ratio of 15:30:1. Meglumine acts as a co-solvent and does not contribute to therapeutic effects. The drug is used primarily for its neuroprotective properties in acute ischemic stroke (AIS), where it has been shown to improve functional outcomes at 90 days compared with placebo. Mechanistically, GDLM exerts multiple effects including maintaining blood-brain barrier integrity, reducing brain edema, improving energy metabolism, antioxidation, anti-inflammation, antiapoptosis, promoting angiogenesis and enhancing blood circulation. It also demonstrates antiplatelet activity and may protect against reperfusion injury by suppressing inflammation and oxidative stress. GDLM is widely used in China for ischemic stroke treatment but is not approved by the US FDA[1][2][5][6][8].
