**Ginsenoside Rb2** is a natural protopanaxadiol-type saponin found in *Panax ginseng* and related ginseng species[1][5][3]. It is a major active component of ginseng extracts and is distributed in roots, stems, and leaves. Rb2 exhibits diverse pharmacological activities in preclinical models, including anti-diabetic, anti-obesity, anti-inflammatory, anti-cancer, antioxidative, anti-osteoporosis, and antiviral actions[1][3][4][7]. Mechanistic studies show Rb2 inhibits oxidative stress, inflammation, and apoptosis by modulating signaling pathways and molecular targets such as AMPK, AKT, MAPK, NF-κB, SIRT1, EGFR/SOX2, TGF-β1/Smad, GPR120/AMPK/HO-1, and miR-216a[1][2][7]. Rb2 promotes insulin sensitivity, glucose uptake, activates brown adipose tissue, and reduces cholesterol and triglyceride levels[1][4][5]. Despite promising preclinical evidence, clinical data for Rb2 as an isolated drug are limited; human studies primarily use ginseng extracts and confirm low bioavailability and slow elimination[1]. No clinical approvals currently exist for any indication.
