A combination drug comprising a **GLP-1 receptor agonist**, an **SGLT2 inhibitor**, and a **dipeptidyl-peptidase 4 (DPP-4) inhibitor** would contain three separate antihyperglycemic agents, each from a distinct and widely used oral/injectable drug class for type 2 diabetes. GLP-1 receptor agonists mimic the incretin hormone GLP-1, stimulate insulin secretion, inhibit glucagon release, delay gastric emptying, and promote weight loss. SGLT2 inhibitors reduce blood sugar by promoting urinary glucose excretion via inhibition of sodium–glucose cotransporter 2 in the renal proximal tubule. DPP-4 inhibitors prolong endogenous incretin activity by preventing breakdown of GLP-1 and GIP and thus enhance glucose-dependent insulin secretion. While dual combinations (such as SGLT2 inhibitor + DPP-4 inhibitor) are available as fixed-dose products, and some triple oral antidiabetic combinations (metformin + SGLT2 inhibitor + DPP-4 inhibitor) exist, there are no known fixed-dose or uniquely branded triple combinations specifically combining a GLP-1 receptor agonist with both an SGLT2 inhibitor and a DPP-4 inhibitor as of November 2025. Clinical guidelines *do not recommend* concurrent use of GLP-1 receptor agonists and DPP-4 inhibitors due to overlapping mechanisms and lack of incremental clinical benefit; thus, any such product would be investigational or conceptual only[1][5][6].
