Overview

The GLP-1 receptor antibody (specifically TB-222-023) is a preclinical-stage monoclonal antibody developed by Twist Bioscience (Twist Biopharma Solutions) that acts as an antagonist of the glucagon-like peptide-1 receptor (GLP-1R). Unlike GLP-1 agonists used for diabetes and obesity, this antibody blocks GLP-1R signaling to reduce excess insulin secretion. The lead optimized candidate, TB-222-023, was derived from an earlier antibody (TB-001-003) using Twist's proprietary Antibody Optimization (TAO) platform. Preclinical studies conducted in collaboration with Children's Hospital of Philadelphia and the University of Pennsylvania demonstrated that TB-222-023 effectively decreases insulin secretion in mouse models and ex vivo human pediatric pancreatic islets, showing greater potency than the clinical-stage peptide antagonist exendin-(9-39). The primary indication being pursued is congenital hyperinsulinism (CHI), with additional potential applications in post-bariatric hypoglycemia (PBH), type 2 diabetes, and neurodegenerative diseases such as Parkinson's and Alzheimer's.
