Overview

A glucagon receptor monoclonal antibody is a type of biologic therapy designed to bind to and inhibit the glucagon receptor (GCGR), a class B G-protein coupled receptor primarily located in the liver. By antagonizing GCGR signaling, these antibodies suppress hepatic glucose output—specifically inhibiting glycogenolysis and gluconeogenesis—which leads to a reduction in blood glucose levels. This mechanism is being explored for the treatment of both type 1 and type 2 diabetes. Beyond glucose lowering, research indicates that blocking the glucagon receptor may promote the regeneration of pancreatic alpha-cells and their conversion into insulin-producing beta-cells, a process potentially mediated by an autocrine GLP-1/GLP-1R loop. Notable clinical and preclinical candidates in this class include Regeneron's REGN1193 and NPB112.
