Overview

Glucose-based peritoneal dialysate is a sterile solution used in peritoneal dialysis for patients with kidney failure. The primary osmotic agent in this fluid is glucose, which draws waste products and excess water from the blood across the peritoneum into the abdominal cavity. After a dwell period, the fluid containing waste is drained and discarded. Glucose remains the standard osmotic agent due to its low cost and metabolic efficiency; however, chronic exposure to high-glucose solutions can lead to adverse metabolic effects such as hyperglycemia, insulin resistance, dyslipidemia, hypertension, overweight/obesity, and increased cardiovascular risk. Prolonged use may also cause local complications including peritoneal fibrosis and angiogenesis[2][4][6]. Glucose absorption from these solutions contributes significantly to caloric load in patients on long-term therapy.
