Glycopyrrolate + formoterol is a fixed-dose combination (FDC) therapy developed by Chiesi Farmaceutici for the treatment of chronic obstructive pulmonary disease (COPD). This combination therapy pairs glycopyrrolate (also known as glycopyrronium bromide), a long-acting muscarinic antagonist (LAMA), with formoterol fumarate, a long-acting beta2-adrenergic agonist (LABA). The dual mechanism of action involves glycopyrrolate's antagonism of M3 muscarinic receptors, which prevents acetylcholine-induced bronchoconstriction, and formoterol's agonism of beta2-adrenoceptors, which promotes bronchodilation by increasing intracellular cyclic AMP. Developed using Chiesi's proprietary Modulite technology, the formulation is typically delivered via a pressurized metered-dose inhaler (pMDI) as an extrafine aerosol, allowing for consistent drug delivery to both the large and small airways. This combination is often evaluated as a component of Chiesi's triple therapy (beclometasone/formoterol/glycopyrronium) or as a standalone dual bronchodilator.
