Overview

Glycopyrronium bromide, also known as glycopyrrolate, is a synthetic quaternary ammonium compound that functions as a competitive muscarinic acetylcholine receptor antagonist. It is primarily used in anesthesia to reduce salivary, tracheobronchial, and pharyngeal secretions, and to protect against the peripheral muscarinic effects of cholinergic drugs. Due to its quaternary ammonium structure, it is highly polar and does not significantly cross the blood-brain barrier, resulting in a lower incidence of central nervous system side effects compared to tertiary amine anticholinergics like atropine or scopolamine. Beyond its perioperative use, glycopyrronium bromide is approved for the treatment of chronic obstructive pulmonary disease (COPD), chronic severe drooling (sialorrhea) in pediatric patients, and primary axillary hyperhidrosis. In the clinical trial ChiCTR2200066599, it is being investigated for the prevention of postoperative nausea and vomiting (PONV) in patients undergoing gynecologic laparoscopic surgery, typically administered in combination with a 5-HT3 receptor antagonist such as tropisetron.
