Overview

GnRH agonist + letrozole is a combination therapy consisting of a gonadotropin-releasing hormone (GnRH) agonist and the aromatase inhibitor letrozole. The GnRH agonist acts by initially stimulating and then downregulating pituitary secretion of luteinizing hormone (LH) and follicle-stimulating hormone (FSH), leading to decreased production of sex hormones such as estrogen and testosterone. Letrozole inhibits the aromatase enzyme, blocking the peripheral conversion of an­drogens to estrogens, further reducing estrogen levels. This combination is used in various clinical settings including fertility preservation in women with estrogen-sensitive cancers, treatment of endometrial carcinoma or complex atypical hyperplasia in young women desiring fertility preservation, management of metastatic male breast cancer, endometriosis suppression prior to assisted reproductive technology cycles, and potentially for polycystic ovary syndrome (PCOS). The combined approach provides more profound suppression of endogenous estrogen than either agent alone[1][2][5].
