Overview

Gold sodium thiomalate is a disease-modifying antirheumatic drug (DMARD) and a gold compound used primarily as an adjunctive treatment for active rheumatoid arthritis in both adults and children. It is administered by intramuscular injection. The drug exerts immunosuppressive and anti-inflammatory effects, though its precise mechanism of action remains unclear. Evidence suggests it may decrease prostaglandin synthesis or alter cellular mechanisms by inhibiting sulfhydryl systems[5][7]. Gold accumulates in synovial cells, leading to dose-dependent inhibition of cell proliferation and protein synthesis, particularly collagen production[6][8]. This suppresses synovitis and slows the progression of joint damage but does not repair existing cartilage or bone damage[1][2][3]. Gold sodium thiomalate is most effective during early active stages of rheumatoid arthritis.
