The GOLP regimen is a multi-modal therapeutic combination developed by Shanghai Zhongshan Hospital for the treatment of intrahepatic cholangiocarcinoma (iCCA). It integrates the GEMOX chemotherapy backbone (gemcitabine and oxaliplatin) with the multi-kinase inhibitor lenvatinib and an anti-PD-1 immune checkpoint inhibitor, such as toripalimab or tislelizumab. Oxaliplatin and gemcitabine function as cytotoxic agents by disrupting DNA replication and transcription. Lenvatinib targets multiple receptor tyrosine kinases, including VEGFR, FGFR, and PDGFR, to inhibit angiogenesis and tumor proliferation. The anti-PD-1 antibody enhances the immune system's ability to recognize and destroy cancer cells. This regimen is primarily investigated as a neoadjuvant or conversion therapy to improve surgical outcomes and survival in patients with high-risk or locally advanced iCCA.
