GP40191 is an ultra-long-acting basal insulin analogue developed by the Russian pharmaceutical company Geropharm as a biosimilar to Novo Nordisk's Tresiba (insulin degludec). It is designed for the treatment of diabetes mellitus, providing a stable and prolonged glucose-lowering effect that lasts beyond 42 hours, which allows for flexibility in dosing timing. The drug functions as an agonist of the insulin receptor, promoting glucose uptake in peripheral tissues such as skeletal muscle and fat, while simultaneously inhibiting hepatic glucose production. GP40191 has undergone comparative clinical trials to demonstrate pharmacokinetic and pharmacodynamic bioequivalence to the reference product and is currently in the registration phase in Russia.
