Overview

GPC3 CAR T cells are an investigational cell-based immunotherapy designed to treat glypican-3 (GPC3)-positive solid tumors, most notably hepatocellular carcinoma (HCC). These therapies involve the genetic engineering of a patient's own T cells to express a chimeric antigen receptor (CAR) that specifically recognizes GPC3, a cell-surface protein highly expressed in HCC but absent in most normal adult tissues. Various iterations are under development, including armored versions that co-express cytokines like interleukin-15 (IL-15) or transcription factors like RUNX3 to enhance persistence and anti-tumor activity within the immunosuppressive tumor microenvironment. Clinical trials have demonstrated early signs of efficacy and manageable safety profiles, with cytokine release syndrome (CRS) being a common but typically controllable side effect.
