GPC3-CART is a chimeric antigen receptor (CAR) T-cell therapy specifically engineered to target Glypican-3 (GPC3), a cell-surface oncofetal antigen that is highly overexpressed in hepatocellular carcinoma (HCC) but has minimal expression in healthy adult tissues. The therapy involves the ex vivo modification of a patient's own T cells to express a CAR that recognizes GPC3, typically incorporating signaling domains such as CD3-zeta and co-stimulatory domains like 4-1BB (CD137) to enhance T-cell activation, proliferation, and persistence. Various iterations of GPC3-CART are under investigation, including second-generation and fourth-generation constructs, the latter of which may include additional elements to modulate the tumor microenvironment. Clinical delivery methods being explored include intravenous infusion, direct intratumoral injection, and transcatheter arterial infusion (TAI) to maximize local cell density at the tumor site and minimize systemic toxicities.
