GPIbα CAAR-Treg cells are a form of engineered cell therapy designed to treat autoimmune diseases, specifically immune thrombocytopenia (ITP). These are human regulatory T cells (Tregs) genetically modified to express a chimeric autoantibody receptor (CAAR) that incorporates the extracellular domain of glycoprotein Ib alpha (GPIbα), fused with intracellular signaling domains such as CD3ζ and 4-1BB. The purpose of this design is to enable the Tregs to selectively recognize and eliminate autoreactive B lymphocytes that produce pathogenic antibodies against GPIbα, a key platelet antigen in ITP. By targeting only these disease-causing B cells while sparing healthy immune function, this approach aims for precise immunosuppression with reduced side effects compared to broad immunosuppressive therapies[1][2]. This therapy is currently at the proof-of-concept stage in preclinical studies.
