GPNMB CAR-T is an experimental second-generation chimeric antigen receptor (CAR) T-cell therapy designed to target Glycoprotein nonmetastatic melanoma protein B (GPNMB). GPNMB is a transmembrane glycoprotein that is highly expressed in glioblastoma (GBM) cells and immunosuppressive tumor-associated macrophages, while showing minimal expression in normal tissues. Developed by researchers at McMaster University and other Canadian institutions, this therapy aims to provide a dual-action approach by eliminating both the malignant tumor cells and the supportive, immunosuppressive niche within the tumor microenvironment. Preclinical studies in patient-derived xenograft (PDX) models have demonstrated that GPNMB CAR-T cells can effectively reduce tumor burden and prolong survival, particularly in recurrent GBM where GPNMB is often upregulated. Research also suggests potential for combinatorial use with other CAR-T targets, such as CD133, to overcome tumor heterogeneity.
