GPO-VIR is a generic fixed-dose combination (FDC) antiretroviral medication developed and manufactured by the Government Pharmaceutical Organization (GPO) in Thailand. It is composed of three active small molecule agents: stavudine (d4T), lamivudine (3TC), and nevirapine (NVP). Stavudine and lamivudine function as nucleoside reverse transcriptase inhibitors (NRTIs), which compete with natural substrates to terminate the DNA chain during viral replication. Nevirapine acts as a non-nucleoside reverse transcriptase inhibitor (NNRTI), binding to an allosteric site on the HIV-1 reverse transcriptase enzyme to induce a conformational change that inhibits its activity. GPO-VIR was a cornerstone of Thailand's National Access to Antiretroviral Programs for People Living with HIV/AIDS (NAPHA), providing a cost-effective, single-tablet regimen for treatment-naïve adult and pediatric patients. While effective in suppressing viral load and increasing CD4 counts, its long-term use has been associated with metabolic and neurological side effects, such as lipodystrophy and peripheral neuropathy.
