GPRC5D bi-specific CAR-T (also referred to as anti-BCMA/GPRC5D bispecific CAR-T) is an investigational autologous chimeric antigen receptor T-cell (CAR-T) therapy designed to simultaneously target two antigens expressed on multiple myeloma cells: G protein-coupled receptor class C group 5 member D (GPRC5D) and B-cell maturation antigen (BCMA). Developed primarily by the Hematology Hospital of the Chinese Academy of Medical Sciences and the Affiliated Hospital of Xuzhou Medical University, the therapy utilizes a single CAR construct incorporating two novel humanized VHH binders. This dual-targeting strategy is intended to reduce the risk of antigen-negative escape, a common cause of relapse in single-target BCMA CAR-T therapies, as GPRC5D and BCMA are independently expressed on myeloma cells. The therapy is currently in Phase 1 clinical development for the treatment of relapsed or refractory multiple myeloma and extramedullary plasmacytoma, with early data indicating deep clinical responses and a manageable safety profile.
