Overview

GPRC5D-targeted CAR-T cell therapy is a type of adoptive cellular immunotherapy designed to treat multiple myeloma. It involves genetically modifying a patient's T cells to express a chimeric antigen receptor (CAR) that specifically recognizes G protein-coupled receptor, class C, group 5, member D (GPRC5D), an orphan receptor highly expressed on malignant plasma cells but with limited expression in normal tissues (primarily hair follicles and keratinized tissues). This therapy is particularly significant for patients who have relapsed after or are refractory to BCMA-targeted therapies. Several candidates are in clinical development, including BMS-986393 (Bristol Myers Squibb) and OriCAR-017. Clinical trials have demonstrated high overall response rates, though unique "on-target, off-tumor" toxicities such as skin rash, nail changes, and dysgeusia are observed due to GPRC5D expression in skin and tongue tissues.
