Overview

GR270773 (Lipidose) is a protein-free phospholipid emulsion originally developed by GlaxoSmithKline for the treatment of suspected or confirmed Gram-negative severe sepsis. The drug's mechanism of action involves the neutralization of bacterial endotoxins (lipopolysaccharides) by enhancing the binding capacity of endogenous lipoproteins, particularly high-density lipoprotein (HDL). By increasing the phospholipid content of these lipoproteins, GR270773 facilitates the sequestration of circulating toxins, which are subsequently cleared by the liver and excreted via the biliary tract. Although it reached Phase II clinical testing in the LIPOS trial, it failed to demonstrate a significant reduction in 28-day all-cause mortality in the overall study population, leading to the discontinuation of its development for sepsis by GSK. Later, the formulation was investigated by Sepsicure for the treatment of chronic endotoxemia in patients undergoing hemodialysis.
