Granulocyte colony-stimulating factor-mobilized peripheral blood CD34+ cells is an autologous cell therapy product consisting of hematopoietic stem and progenitor cells (HSPCs) that have been mobilized from the bone marrow into the peripheral circulation using G-CSF and subsequently enriched for the CD34 surface marker. In the context of ischemic stroke, these cells are administered to promote functional recovery through a multi-faceted mechanism. Upon administration, the cells home to the ischemic brain tissue via the CXCR4/SDF-1 signaling axis. Once at the site of injury, they exert therapeutic effects primarily through paracrine signaling, secreting various growth factors such as vascular endothelial growth factor (VEGF), brain-derived neurotrophic factor (BDNF), and hepatocyte growth factor (HGF). These factors stimulate angiogenesis, neurogenesis, and synaptogenesis while modulating the local inflammatory environment, thereby enhancing the brain's endogenous repair mechanisms following an acute or subacute ischemic event.
