Overview

Granulocyte colony-stimulating factor-primed donor lymphocytes (GP-DLI) is a cellular immunotherapy product used primarily in the context of allogeneic hematopoietic stem cell transplantation (allo-HSCT). The therapy involves treating healthy donors with G-CSF (such as filgrastim) prior to the collection of lymphocytes via apheresis. This priming process modifies the immunological characteristics of the collected lymphocytes, inducing a shift from a Th1 to a Th2 immune response, increasing the proportion of regulatory T cells, and modulating dendritic cell function. These modifications are intended to induce T-cell immune tolerance, thereby reducing the incidence and severity of graft-versus-host disease (GVHD) while preserving the potent graft-versus-leukemia (GVL) effect. GP-DLI is frequently utilized as part of the 'Beijing Protocol' for haploidentical transplantation to prevent or treat relapse in patients with high-risk hematological malignancies, including acute myeloid leukemia and acute lymphoblastic leukemia.
