Overview

Granulocyte concentrate is a **blood product** prepared by collecting granulocytes (primarily **neutrophils**) from healthy donors, typically via **apheresis**. The donor is often pretreated with **granulocyte colony-stimulating factor (G-CSF)** and/or corticosteroids (such as dexamethasone) to mobilize and increase the yield of granulocytes in the collection. Each concentrate contains a high number of granulocytes (at least 1×10¹⁰ per unit), suspended in plasma, and also includes other blood cells, such as red blood cells, platelets, and other leukocytes. The product is irradiated prior to transfusion to prevent transfusion-associated graft-versus-host disease (TA-GVHD). Granulocyte concentrate is primarily used as a **supportive therapy** for patients with **prolonged, severe neutropenia** (absolute neutrophil count <500/μL) or documented neutrophil dysfunction who have severe, refractory bacterial or fungal infections that do not respond to antimicrobial therapy. It serves as a temporary measure ("bridge") until the patient's bone marrow recovers. The efficacy of granulocyte concentrate therapy remains controversial, with limited clinical data demonstrating survival benefit. The product must be transfused shortly after preparation due to granulocyte fragility and short shelf life[1][3][5][6][7][8].
