Grisnilimab setaritox (PHL001) is an immunotoxin being developed by Philikos B.V. for the treatment of diffuse cutaneous systemic sclerosis (dcSSc). It is composed of grisnilimab, a humanized monoclonal antibody targeting the CD19 antigen expressed on B cells, chemically conjugated to setaritox, which is the A chain of the toxin ricin. Upon binding to CD19, the conjugate is internalized via receptor-mediated endocytosis. The ricin A chain is then released into the cytosol, where it enzymatically inactivates the 28S ribosomal RNA by depurinating a specific adenine residue. This action halts protein synthesis and induces apoptosis in the targeted B cells. This B-cell depletion strategy is intended to reduce the production of autoantibodies and pro-fibrotic cytokines that drive the pathogenesis of systemic sclerosis, potentially halting or reversing the fibrotic process.
