The GRPR theranostic pair is a radiopharmaceutical program being developed by Fuzionaire Radioisotope Technologies (formerly Fuzionaire Diagnostics) that targets the gastrin-releasing peptide receptor (GRPR) for both imaging and treatment of cancer. The program utilizes the company's proprietary HetSiFA (heteroaromatic silicon-fluoride acceptor) platform technology to create chemically identical diagnostic and therapeutic pairs. For imaging, the program uses fluorine-18 (18F) and gallium-68 (68Ga) radiolabeled ligands for PET scanning, while for therapy it employs lutetium-177 (177Lu), terbium-161 (161Tb), and actinium-225 (225Ac) radionuclides for targeted radioligand therapy. The primary indications are metastatic prostate cancer and ER-positive breast cancer, with GRPR serving as a complementary target to the widely used PSMA target in prostate cancer. The program is currently in preclinical development, with plans for first patient tests under compassionate use laws in Germany.
