GRT-C903 + GRT-R904 + KRAS TCR-transduced PBL is an investigational combination immunotherapy regimen being developed by the National Cancer Institute (NCI) in collaboration with Gritstone Bio. The treatment is designed for patients with metastatic solid cancers harboring KRAS mutations, such as colorectal, pancreatic, and non-small cell lung cancers. The regimen employs a "prime-boost" vaccination strategy: GRT-C903 is a chimpanzee adenoviral (ChAd) vector vaccine used for priming the immune system, and GRT-R904 is a self-amplifying mRNA (samRNA) vaccine delivered in lipid nanoparticles used for boosting. Both vaccines deliver shared KRAS neoantigens. This vaccination is combined with the adoptive transfer of autologous peripheral blood lymphocytes (PBL) that have been genetically engineered to express T-cell receptors (TCRs) specifically reactive against KRAS mutations. The goal of this multi-pronged approach is to generate and expand a robust population of KRAS-specific cytotoxic T-lymphocytes to eliminate tumor cells.
