GS-5718-03 is an orally bioavailable, small molecule inhibitor of Interleukin-1 receptor-associated kinase 4 (IRAK4), currently under development by Gilead Sciences for the treatment of autoimmune disorders. IRAK4 is a critical mediator in the signaling pathways of the Interleukin-1 receptor (IL-1R) family and various Toll-like receptors (TLRs), which play a central role in the innate immune response. By selectively inhibiting IRAK4, GS-5718-03 prevents the activation of downstream transcription factors such as NF-κB and MAP kinases, thereby reducing the production of pro-inflammatory cytokines and Type I interferons. This mechanism is particularly relevant for systemic lupus erythematosus (SLE) and cutaneous lupus erythematosus (CLE), where dysregulated TLR signaling and interferon signatures are key drivers of disease pathology.
