Overview

GSK315234 is a humanized monoclonal antibody developed by GlaxoSmithKline (GSK) for the treatment of rheumatoid arthritis (RA). It targets and inhibits Oncostatin M (OSM), a cytokine that plays a role in inflammation and joint damage in RA patients. The drug was investigated in clinical trials, including a Phase II study, conducted primarily in the UK and Europe. It was administered via intravenous infusion (e.g., 3 mg/kg) and also tested as a subcutaneous injection. Clinical trials indicated good tolerability, with common adverse events including aggravation of RA, elevated alanine aminotransferase, fever, headache, hypertension, and diarrhea. Despite some statistically significant differences in DAS28 scores compared to placebo in certain trials, the overall therapeutic efficacy of GSK315234 was considered not very satisfactory and limited. Its modest effectiveness was attributed to its moderate binding affinity and rapid off-rate for OSM, which led to a 'protein carrier effect' where the OSM/antibody complex accumulated, prolonging OSM's half-life in serum and synovial fluid. This suggested a need for an anti-OSM monoclonal antibody with higher affinity for effective RA treatment.
