Vera Salus Ricerca is developing a series of small molecule agonists targeting Guanylate cyclase 2C (GUCY2C), also known as Guanylyl cyclase C (GC-C), for the treatment of gastrointestinal disorders. GC-C is a transmembrane receptor primarily expressed on the apical surface of intestinal epithelial cells. Activation of this receptor by small molecule agonists mimics the action of endogenous peptides like guanylin and uroguanylin, leading to an increase in intracellular cyclic guanosine monophosphate (cGMP). This signaling cascade activates the cystic fibrosis transmembrane conductance regulator (CFTR), resulting in the secretion of chloride and bicarbonate into the intestinal lumen, which increases fluid secretion and accelerates gastrointestinal transit. Furthermore, elevated extracellular cGMP levels are thought to inhibit the activity of visceral pain-sensing nerves, potentially providing relief from abdominal discomfort. The program is currently in the preclinical stage of development.
