Guben Huoxue Tongluo formula (GBHXTLF) is a multi-component Traditional Chinese Medicine (TCM) decoction developed by the Teaching Hospital of Chengdu University of Traditional Chinese Medicine specifically for the treatment of diabetic kidney disease (DKD). The formula is designed based on the TCM therapeutic principles of 'securing the root' (addressing underlying Qi and Yin deficiencies), 'activating blood' (improving microcirculation), and 'dredging collaterals' (resolving stasis in the renal vasculature). It typically comprises a standardized mixture of herbs including *Astragalus membranaceus* (Huangqi), *Rehmannia glutinosa* (Dihuang), and *Salvia miltiorrhiza* (Danshen). GBHXTLF exerts its therapeutic effects by mitigating renal fibrosis, reducing oxidative stress, and inhibiting inflammatory responses. Clinical and preclinical studies suggest it works by modulating the TGF-β1/Smad signaling pathway to prevent extracellular matrix accumulation and activating the Nrf2/HO-1 antioxidant pathway to protect podocytes and tubular cells from glucose-induced damage.
