GV20-0251 + pembrolizumab is an investigational combination therapy for advanced solid tumors, currently in clinical trials. GV20-0251 is a first-in-class Fc-silenced IgG1 monoclonal antibody that targets the novel immune checkpoint Immunoglobulin superfamily member 8 (IGSF8). By inhibiting IGSF8, it is designed to enhance natural killer cell-mediated cytotoxicity, upregulate dendritic cell antigen presentation, and increase T cell signaling. Pembrolizumab (Keytruda) is a well-established anti-Programmed cell death protein 1 receptor (PD‑1) immune checkpoint inhibitor that blocks the programmed death receptor 1 pathway to promote T-cell mediated antitumor activity. The combination aims to provide synergistic immunomodulation by targeting both innate and adaptive immune checkpoints. This regimen is being developed primarily for patients with advanced or refractory solid tumors who have failed standard therapies, including indications such as endometrial carcinoma, melanoma, non-small cell lung cancer, rectal adenocarcinoma, squamous cell carcinoma of head and neck, and other solid malignancies[2][3][4][5][6][8].
