GW863 is a peripherally selective kappa opioid receptor (KOR) agonist developed by Chengdu Gowell Pharmaceutical for the treatment of chronic kidney disease-associated pruritus (CKD-aP). By specifically activating KORs on peripheral sensory nerve endings and immune cells, GW863 inhibits the transmission of pruritic signals and the release of inflammatory mediators. Its peripheral selectivity is designed to prevent the drug from crossing the blood-brain barrier, thereby avoiding central nervous system-mediated side effects such as sedation, dysphoria, or hallucinations typically associated with non-selective opioid agonists. GW863 is currently in Phase 3 clinical development in China, primarily targeting patients with end-stage renal disease who experience moderate-to-severe pruritus while undergoing hemodialysis.
