Gymnemantoside A is a newly identified methyl anthranilate-conjugated oleanane triterpenoid isolated from the medicinal plant *Gymnema inodorum*, which is indigenous to Thailand. It is being investigated for its potential to treat skeletal muscle atrophy, particularly forms induced by glucocorticoids (steroids), aging (sarcopenia), or disease (cancer cachexia). The compound functions through a dual mechanism: it competitively inhibits the glucocorticoid receptor (GR) to prevent steroid-induced muscle wasting and simultaneously activates the insulin receptor (IR) tyrosine kinase domain. This activation restores the Akt/mTOR-mediated insulin signaling pathway, increases the expression of insulin-like growth factor-1 (IGF-1), and downregulates the expression of atrogenes such as atrogin-1 and MuRF-1. Preclinical studies in murine models have demonstrated that gymnemantoside A can recover muscle fiber cross-sectional area and significantly enhance exercise endurance.
