Overview

GZR4 is a fourth-generation, ultra-long-acting insulin analog designed for once-weekly subcutaneous administration. Developed independently by Gan & Lee Pharmaceuticals, it is intended for the treatment of diabetes mellitus, particularly type 2 diabetes. The molecular design of GZR4 includes three amino acid substitutions in the human insulin backbone (A14E, B16H, B25H), removal of the B30 threonine residue from the insulin B-chain, and conjugation of a 22-carbon fatty diacid to the B29 position via a 12-unit oligomeric ethylene glycol spacer. This structure confers high affinity for human serum albumin and an extended half-life (~135 hours at steady state), enabling stable basal glycemic control with once-weekly dosing. Clinical trials have demonstrated that GZR4 provides effective glucose-lowering comparable or superior to daily long-acting insulins such as insulin degludec (IDeg), with favorable safety and tolerability profiles and no severe hypoglycemia reported[1][5][6][7][8].
