H2CasMab-2 is a cancer-specific monoclonal antibody (CasMab) targeting the human epidermal growth factor receptor 2 (HER2). Developed by researchers at Tohoku University, it is designed to bind specifically to a locally misfolded region of HER2 that is present on cancer cells but absent or inaccessible on normal tissues. This cancer-specific binding profile aims to reduce off-target adverse effects compared to conventional anti-HER2 therapies. Preclinical evaluations of various versions, including the humanized IgG1 form (humH2CasMab-2), have demonstrated significant antitumor efficacy in breast cancer xenograft models. While it exhibits lower antibody-dependent cellular cytotoxicity (ADCC) than trastuzumab, it shows significantly higher complement-dependent cytotoxicity (CDC), suggesting a distinct immunological mechanism for its therapeutic effect.
