h2E2 (also known as IXT-m200) is a recombinant humanized monoclonal antibody (IgG1) developed as a passive immunotherapy for cocaine-use disorder and cocaine toxicity. It is designed to bind specifically to cocaine with high affinity (Kd of approximately 4 nM) and high selectivity, avoiding interaction with inactive metabolites like benzoylecgonine. The antibody functions by sequestering cocaine within the plasma compartment, effectively preventing the drug from crossing the blood-brain barrier. By limiting cocaine's access to the central nervous system, h2E2 blocks the drug's ability to inhibit dopamine reuptake, thereby neutralizing its reinforcing effects and psychoactive properties. The antibody has a 95% sequence homology with human IgG1 and is produced in transfected mammalian cell lines.
