The H3K27M peptide vaccine (H3K27M-vac) is an investigational therapeutic cancer vaccine developed by the German Cancer Research Center (DKFZ) for the treatment of H3K27M-mutant diffuse midline gliomas (DMG). The vaccine consists of a 27-amino acid peptide spanning positions 14-40 of the histone H3 protein, specifically containing the lysine-to-methionine substitution at position 27 (H3K27M). It is designed to induce a mutation-specific, MHC class II-restricted CD4+ T cell response against the neoepitope present in the tumor cells. Typically formulated with the adjuvant Montanide ISA50 and supplemented with topical imiquimod, the vaccine is currently being evaluated in the Phase I INTERCEPT-H3 trial in combination with radiotherapy and the checkpoint inhibitor atezolizumab.
