Overview

Haplo-cord HCT is a combination hematopoietic cell transplantation procedure that involves the coinfusion of stem cells from both an unrelated umbilical cord blood (UCB) unit and a haploidentical (half-matched) donor, typically a family member. The approach leverages the rapid engraftment potential of CD34-selected cells from the haploidentical donor and the durable engraftment and graft-versus-leukemia effect provided by UCB progenitors. This method is primarily used to treat hematological malignancies such as acute myeloid leukemia (AML), lymphomas, and aplastic anemia in patients lacking fully matched donors. Clinical trials have demonstrated improved overall survival rates compared to standard haplo-HCT alone, with faster neutrophil recovery and enhanced immune reconstitution without excessive adverse events[1][2][3][5]. The mechanism relies on restoring healthy hematopoiesis through allogeneic stem cell infusion while reducing relapse risk via immunologic effects from both graft sources.
