Overview

Haplo-Cord Transplant is a specialized allogeneic hematopoietic stem cell transplantation (HSCT) protocol that combines a haploidentical (half-matched) donor graft with an umbilical cord blood (UCB) unit. Developed and pioneered at the University of Chicago, this approach is designed to overcome the limitations of both individual graft types: the haploidentical graft provides rapid, early myeloid engraftment to reduce the period of post-transplant neutropenia, while the cord blood unit provides long-term, durable hematopoiesis and a potent graft-versus-leukemia (GVL) effect. In the context of the Rapid Study (NCT02743819), this protocol is being evaluated as a strategy to expedite the transplant process for patients with acute myeloid leukemia (AML) and high-risk myelodysplastic syndromes (MDS) who lack a matched sibling donor, comparing the time-to-transplant against the standard search for a matched unrelated donor (MUD). The haploidentical portion of the graft may undergo selection or processing using specialized devices, such as those from Miltenyi Biotec, to optimize the cell composition.
