This intervention is a specialized cellular therapy protocol developed by the Masonic Cancer Center at the University of Minnesota for the treatment of primary or secondary graft failure in patients who have previously undergone hematopoietic stem cell transplantation (HSCT). The treatment consists of a non-myeloablative conditioning regimen followed by a hematopoietic stem cell infusion. The conditioning regimen includes fludarabine, cyclophosphamide, and low-dose total body irradiation (TBI), designed to suppress the recipient's immune system and facilitate donor engraftment. The graft itself consists of bone marrow or G-CSF mobilized peripheral blood stem cells obtained from a haploidentical related donor. The primary goal of this therapeutic approach is to restore hematopoiesis and achieve donor engraftment by day 42 post-infusion in patients for whom initial transplantation failed.
