Overview

Haploidentical hematopoietic stem cells are blood-forming stem cells collected from a donor who is a partial genetic match (haploidentical) to the recipient. Typically, these donors are first-degree relatives—such as parents or children—who share exactly half of their human leukocyte antigen (HLA) markers with the patient. The use of haploidentical donors expands the pool of potential donors for patients needing allogeneic hematopoietic stem cell transplantation, especially when no fully HLA-matched donor is available[1][4][8]. These transplants are primarily used to treat various blood cancers (hematologic malignancies), bone marrow failure syndromes, and certain inherited disorders such as sickle cell disease and severe combined immunodeficiency[1][4][8]. The mechanism involves replacing diseased or dysfunctional bone marrow with healthy donor-derived hematopoietic progenitor cells that can reconstitute normal blood and immune function. Advances in conditioning regimens and post-transplant immunosuppression have improved outcomes by reducing graft-versus-host disease while maintaining graft-versus-tumor effects[5].
