This investigational combination immunotherapy consists of adoptively transferred haploidentical allogeneic natural killer (NK) cells and the PARP inhibitor talazoparib. Developed by the German Cancer Research Center (DKFZ), the therapy is specifically designed to target leukemic stem cells (LSCs) in patients with acute myeloid leukemia (AML). The mechanism involves talazoparib-induced upregulation of NKG2D ligands on LSCs, which are typically downregulated as an immune evasion tactic. This sensitization makes the LSCs susceptible to elimination by the infused allogeneic NK cells. The treatment is currently being evaluated in the Phase I/II NAKIP-AML trial for patients with relapsed/refractory or minimal residual disease (MRD)-positive AML.
