Overview

Haploidentical peripheral blood stem cells (haplo-PBSC) are hematopoietic stem cells collected from the peripheral blood of a donor who is a half-match (haploidentical) for the recipient's human leukocyte antigens (HLA). These cells are used in allogeneic hematopoietic stem cell transplantation (allo-HSCT) for patients with high-risk hematologic malignancies, such as acute leukemia, who lack a fully HLA-matched donor. The procedure typically involves mobilizing stem cells from the donor's bone marrow into the peripheral blood using growth factors like granulocyte colony-stimulating factor (G-CSF), followed by apheresis to collect the cells. Because of the HLA mismatch, specialized graft-versus-host disease (GVHD) prophylaxis, such as post-transplant cyclophosphamide (PTCy) or anti-thymocyte globulin (ATG), is often required to prevent severe immune reactions while maintaining the graft-versus-leukemia effect. This approach has significantly expanded the donor pool, as most patients have at least one haploidentical family member.
