hAT-MSC.sTRAIL is an engineered cell therapy consisting of human adipose-derived mesenchymal stem cells (hAT-MSCs) modified to express and secrete a soluble form of the tumor necrosis factor-related apoptosis-inducing ligand (sTRAIL). This therapeutic approach utilizes the innate tumor-homing properties of mesenchymal stem cells to deliver sTRAIL directly to the tumor microenvironment, particularly in difficult-to-treat malignancies such as brainstem glioma. Once at the tumor site, the secreted sTRAIL binds to death receptors 4 (DR4) and 5 (DR5) on the surface of cancer cells, triggering the extrinsic apoptotic pathway. Research conducted at Seoul National University Hospital has explored this agent in combination with histone deacetylase (HDAC) inhibitors like panobinostat to overcome TRAIL resistance by upregulating death receptor expression and enhancing the induction of both apoptosis and autophagy in glioma cells.
