HB134 is a bispecific T-cell engager (TCE) targeting leukocyte immunoglobulin-like receptor B4 (LILRB4) and CD3, developed by Huahui Health for the treatment of monocytic acute myeloid leukemia (AML). The molecule is engineered by fusing an anti-LILRB4 nanobody and an anti-CD3 Fab to a human IgG1 scaffold with a silent Fc region. LILRB4 is a specific marker overexpressed in monocytic AML subtypes (M4 and M5) but absent in normal hematopoietic stem cells, providing a therapeutic window. HB134 works by recruiting T cells to LILRB4-positive leukemia cells, inducing potent T-cell activation and tumor cell cytotoxicity while maintaining a favorable safety profile with low induction of proinflammatory cytokines in preclinical studies.
