Overview

Hematopoietic Cell Transplantation-Stanford University-systemic lupus erythematosus refers to a specific therapeutic procedure developed at Stanford University for patients with severe, refractory systemic lupus erythematosus (SLE). The protocol (NCT00325741) utilizes an allogeneic hematopoietic cell transplantation (HCT) approach with a non-myeloablative conditioning regimen. This regimen consists of total lymphoid irradiation (TLI) and rabbit anti-thymocyte globulin (ATG) to deplete the host's autoreactive immune cells without completely destroying the bone marrow. Following conditioning, patients receive an infusion of HLA-matched donor CD34+ hematopoietic stem cells. The goal is to establish stable mixed chimerism, where donor-derived immune cells replace the patient's dysfunctional immune system, potentially leading to long-term remission of life-threatening SLE manifestations in the kidneys, lungs, heart, or central nervous system.
