Overview

This combined cell therapy approach, developed by the University of Zurich, involves the transplantation of peripherally mobilized, donor-derived CD34+ hematopoietic stem cells in conjunction with a kidney transplant for patients with end-stage renal disease (ESRD). The protocol utilizes HLA-identical sibling donors and includes a conditioning regimen of anti-thymocyte globulin (ATG) and total lymphoid irradiation (TLI). The primary mechanism of action is the induction of mixed lymphohematopoietic chimerism, which facilitates immunological tolerance in the recipient toward the donor kidney. This strategy is designed to allow for the eventual withdrawal of standard immunosuppressive medications while maintaining stable graft function, potentially offering a functional cure for ESRD by avoiding the long-term toxicities associated with chronic anti-rejection therapy.
